Position Description/Summary:


Provides intensive home visiting services for youth who are active with CFS. Youth Mentors spend time with youth engaging them in positive activities and provide them with learning opportunities and positive role model.

Duties & Responsibilities:
 The Youth Mentor Program is responsible for providing direct client services that promote the functioning and wellbeing of the youth.

  • Provide direct client services as directed by the CFS worker.
  • Complete appropriate documentation for services provided including service reports and billing.
  • Assist clients in meeting safety plan goals set by the family.
  • Communicate with case workers regarding client progress.
  • Verify authorization for services provided.
  • Build winning relationships with clients.
  • Ability to use technology to communicate effectively.
  • Ability to meet weekly and monthly deadlines consistently.

    • Requirements:

  • Valid Class 5 Driver’s License (or other means of transportation).
  • Must provide their own transportation for which they will submit mileage log for reimbursement.
  • Must be able to provide a recent and satisfactory Criminal Record check, Driver’s Abstract and Child Abuse Record check.
